Case Manager Lead-Full TIme Days

The lead Case manager promotes optimal health care outcomes in accordance with the policies, procedures, applicable laws and contracts, philosophy, objectives and mission of West Suburban Medical Center. Coordinates care in collaboration with multidisciplinary team to provide and facilitate discharge planning as well as utilization of hospital resources. Maintains confidentiality of patient's health information.Actively supports organizational goals and objectives by providing needed information to divisions and departments. The lead CM acts as a resource to the NCMs and SWs by providing education related to the Revenue Cycle, CMS standards, insurance authorization/clinical denial concerns.

Qualifications:


  1. Registered Nurse with current Illinois License, (Master Degree preferred)

  2. Minimum of three years of clinical experience in the area of clinical speciality.

  3. Background/experience in Utilization Management or Case Management supervisory preferred.

  4. Case Management certification required

  5. Proficient in Midas/All Scripts. Proficient in Microsoft Outlook/Excel/Word

  6. Working knowledge of CMS regulations related to CM.

  7. Understanding of the Revenue Cycle.

  8. Incorporates current Evidence-based practice to maximize positive outcomes related to CM model.

  9. Basic understanding of VBP and service standards related to Patient Experience.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
