Adult & Adolescent Programming Coordinator, QIDP

PACTT Learning Center PACTT Learning Center Oak Park, IL Oak Park, IL Full-time Full-time $52,500 a year $52,500 a year The Adult & Adolescent Programming Coordinator is a member of a team of professionals who support PACTT's Oak Park adult and adolescent participants.
The AP Coordinator will develop, plan, and deliver quality, life-enriching job experiences as well as arts and recreation programs for PACTT individuals living in our Oak Park and Elmwood Park homes and who are part of our vocational services or live in our Children's Group Homes.
The goal of this position is to enhance the opportunities available to PACTT individuals during their day program, including leisure activities, work and community participation.
Essential Functions:
1.
Services:
Support our adults living in our Oak Park homes and attending our Oak Park day program in providing a wide array of working, arts, recreation and leisure opportunities that work to enhance each individual's unique communication and social interaction skills.
Attend all appropriate staff meetings in order to facilitate integration and coordinator of activities.
Provide training to PACTT employees specific to participant and community program involvement.
Work with the Oak Park team to develop meaningful on-site and off-site recreational and arts experiences to be integrated into day and evening programming.
Work on-site with the team to help deliver home-based life skills, recreational and art experiences.
2.
Assessment:
Participate in ongoing assessment, with teams focusing on job, recreation and leisure interests of the individual's served, and creation of plans to address those interests.
3.
Collaboration:
Collaborate with employers, special recreation and park district providers and other community programs/agencies offering employment, social, art, recreation, sport, or educational activities that are of interest to PACTT participants.
Collaboration includes but is not limited to helping provide outside agencies the appropriate training and information regarding participants' needs; assist outside agencies in planning for successful integration; performing ongoing assessment and development of programming.
4.
QIDP:
Completion of paperwork including Q-notes, assessments associated with the implementation strategies, and other documentation associated with QIDP responsibilities.
Nonessential Job Functions 1.
Provide direct service to participants.
Includes but not limited to habilitative services,assistance with ADLs, community integration, home tasks, etc.
These activities may be done in the community or on site at PACTT locations.
Qualifications:
1.
Bachelor's degree from accredited college or university, preferably in the area of social service (psychology, social work) or arts/recreation (therapeutic recreation, art therapy, music therapy), strongly preferred.
2.
QIDP qualified.
3.
At least 3 years' experience in direct service of individuals with disabilities.
4.
Skill in dealing with individuals with emotional and behavior challenges.
5.
Physical and emotional capability to implement emergency crisis procedures, and ensure individual, staff and community members' safety through implementation of designed interventions.
6.
Ability to communicate effectively and develop cooperative relationships with families, co-workers, and others; and to professionally articulate knowledge and skills related to the job.
7.
Ability to work and communicate effectively with a wide range of individuals and stakeholders.
8.
Ability to learn individuals' communication systems.
9.
Ability to prepare accurate, complete records.
10.
Ability to plan ahead and to cope with flexible and variant environment.
11.
Intermediate computer skills, including competence with Microsoft products (Word, Excel, Power Point, Outlook, etc.
) 12.
Familiarity with Boardmaker and similar software to create social stories, choice making boards, and schedules as needed, preferred.
